Outreach

The PHS Integrated Housing Team is an outreach team that operates 365 days a year supporting residents that live in both PHS and Atira Property Management buildings in Vancouver’s Downtown Eastside.

The team supports a highly marginalized community of residents with the aim of assisting people to improve their health, maintain their tenancy and achieve any goals they may have by connecting them with community resources.

Being onsite daily allows the Integrated Housing Team to remain in constant contact with residents’ clinical care teams and legal representatives to ensure appointments are met (often accompanied), medications are administered correctly and issues are addressed immediately. The team also enables individuals to work towards long-term goals relating to housing, harm-reduction and healthcare, along with accessing social services, legal or financial supports.

The team supports residents after they leave the properties to ensure a productive and supported transition – until these supports are no longer needed or other community resources have been found. Regular program outings and events help bring a sense of community among disenfranchised tenants.

The Integrated Housing Team works closely with the Vancouver Coastal Health Primary Outreach Services Hotel Team and Columbia Street Community Clinic to provide onsite healthcare for those unable to access community clinics.
